Electronic Medical Records (EMR)

With EMR features such as dictation templates, laboratory support, risk factor analysis routines, comprehensive medication management, and customizable medical reports and graphs; the Team Chart Concept allows you to practice medicine faster, more accurately, and to a higher standard of care than was possible with paper charts. Instead of using a paper chart, the medical staff documents all clinical information in the Team Chart Concept. Provider dictation, vital signs, prescriptions, lab results, photographs, and more are stored in one location and allow for rapid entry and retrieval.

TCC's dictation templates offer the provider an unparalleled ability to rapidly enter all information related to a patient encounter.

Laboratory
Healthcare providers often need to quickly request and/or access laboratory information. With TCC, the provider can at any time:
• Request Lab Orders
• View previous lab results
• Create reports and graphs based on lab results

In addition, the Team Chart Concept offers extensive support for inside and ouside laboratory equipment and services. TCC uses industry standard protocols such as HL7 to import data from outside laboratories. The imported information is then placed directly in the patient's electronic medical record.

 
       

Photographs
The Team Chart Concept also stores photographs, sound files, and even video files. Patient images may be used to document skin conditions, cases of abuse, or disease progression. Patient photographs can even be printed on the prescriptions for security.

• By illustrating personal risk for disease, TCC's Risk Factor Analysis routines give the provider a powerful educational tool to use on behalf of the patient.

Disease Management
One important distinction of the Team Chart Concept compared to traditional electronic medical documentation systems is its ability to apply relevant medical research outcomes to routine clinical practice in the community.

For example, aggressive monitoring is essential in managing diabetes successfully. Glucose levels, lipid levels, blood pressure, peripheral sensory and vascular function, ophthalmic evaluation, and kidney function should be evaluated regulary.
